Both offices accept most all traditional, HMO and PPO health care plans. The office staff will bill traditional insurance plans and Medicare for you.

Please notify our office immediately if you change insurance policies. Please notify us if another physician changes your medications or you are scheduled for a surgery.

For HMO and PPO patients, one of our physicians can act as your Primary Care Physician or PCP. In that role he will provide all of your general medical care, including:

  • Routine Office Visits
  • Immunizations
  • Diagnostic X-rays
  • Gynecologic exams and routine Pap smears
  • Allergy Injections
  • Suturing of minor lacerations
  • Minor surgery
  • Laboratory Tests

Because Dr. Johnson and his staff provide such a broad spectrum of care, we prefer to manage your medical needs through our offices as much as possible. This will ensure that your care is handled by a medical team that knows your needs and medical history.

If you think you need a referral to a medical specialist, such as a cardiologist, you must first visit one of our physicians. He will determine your medical needs, then (if appropriate) write a referral form and direct you to the correct specialist. A referral form must be obtained before you visit a specialist. Referral forms will be generated within five working days of your visit. If pre-authorization is not obtained, you may be responsible for payment of the insurance claim.
